Under Eye Fat Grafting Cost Korea: Hollow or Bag? Reviews, Results and Recovery

Under-eye fat grafting adds your own fat to a true hollow. It does not fix a bulging bag. What it costs comes down to the cause, how many areas are treated, the anesthesia and whether a second session is planned.

The short answer

Under eye fat grafting cost Korea: is it worth paying for when you aren't sure the dark groove under your eyes is a hollow at all? The figure a clinic quotes follows what causes the shadow, how much fat goes where, the anesthesia and whether a second session is planned. If you live abroad, settle the cause first. Flying to Seoul for the wrong operation costs far more than the operation itself.

  • The cost follows the cause of the shadow: a hollow, a bag, or both.
  • Fat grafting adds volume to a hollow. It does not fix a bulging bag.
  • Part of the graft is reabsorbed, so the contour after a few months is closer to what stays.
  • Foreign patients pay the same surgery fee as Korean patients, with a written estimate.
  • Ask how many days to plan before you book the return flight.

After that come four answers: what moves the cost, which Reddit claims hold up, how much of the result stays, and what recovery means for your trip. Together they should help you tell whether grafting, repositioning or both fit the face you see in the mirror.

Hello, I'm Dr. Im Young Min, Chief Director and board-certified plastic surgeon at VG Plastic Surgery in Gangnam, Seoul. I trained at Seoul St. Mary's Hospital, where I'm an adjunct professor of plastic surgery. I'm also a full member of the American Society for Aesthetic Plastic Surgery.

Three anesthesiologists work here on staff, and one is dedicated to every operation. They don't perform cosmetic surgery. Foreign patients get a medical interpreter for their language at no extra charge.

Most people price the graft first. Only later do they learn the shadow comes from a bag, and fat can't hide a bag.

If a bulging fat pad casts your shadow, I won't recommend grafting fat on top of it. Fat grafting is for a true hollow, a groove where volume is missing, and that is where I'd consider it.

Before you compare figures, send one photo on WhatsApp: your face straight on, in daylight, eyes looking ahead, with no makeup under the eyes. Ask one thing with it: hollow, bag, or both? A daylight photo shows where the shadow actually sits, and every cost figure starts from that answer.

Hollow or bag: which one is under your eyes?

Stand facing a window with a mirror in your hand, then tilt your chin down slightly. A bag is a bulge. The fat that normally cushions the eye pushes forward through a weakening lid, and it often stands out more when you look up. A hollow is the opposite: a dip along the bony rim where volume has thinned. It looks darker under overhead light. Many people over 40 have both, a bulge with a groove underneath.

What you seeWhat's usually behind itWhat's usually discussed
A dip along the rim, no bulgeVolume loss over the boneFat grafting
A bulge that grows when you look upFat pad pushing forwardLower lid fat repositioning or removal
A bulge with a groove below itBothRepositioning, sometimes with grafting
Dark color, flat contourPigment or thin skinOften not surgery at all

The last row matters. A shadow that is mostly pigment won't change much with added volume. I'd say so rather than plan surgery around it. For bags, the American Society of Plastic Surgeons describes lower eyelid surgery as removing or redistributing excess fat. If your mirror test shows a bulge, read about how lower lid fat repositioning handles bags and the groove beneath them next.

Say you're 48. The groove running from your inner corner down onto the cheek has deepened over five years, and you've had filler there twice. The last round left the area a little puffy, with a faint bluish tint in some light. Before anything else, I'd want to know whether that puffiness is leftover filler. Hyaluronic acid filler under thin lower lid skin can spread or hold water and end up looking like a bag. Grafting fat over it would stack one volume on top of another.

In that situation I'd talk about dissolving the remaining filler and letting the area settle before deciding on fat. The US Food and Drug Administration lists swelling, lumps and, rarely, injection into a blood vessel among the known risks of dermal fillers. That is one more reason not to layer treatments blindly.

How does fat grafting under the eyes work?

Fat grafting moves a small amount of your own fat from somewhere you have spare, often the lower abdomen or inner thigh, into the hollow. The fat is drawn out through a thin cannula, purified, then placed in fine threads at more than one depth. Our homepage sums up the goal in one line: what counts is not how much went in, but how much of it stayed. That matters more under the eyes than anywhere else, because the skin is thin and a small excess shows as a lump.

Not all of the transplanted fat survives the move. Cells that pick up a blood supply in the first weeks tend to stay as living tissue, and the rest is reabsorbed. So overfilling a hollow on the first pass, hoping some will disappear, is a poor trade under the eyes. Too much fat that survives is harder to correct than too little.

If you're also thinking about your temples or cheeks, see which face areas are grafted and how much fat tends to take.

Under eye fat grafting cost Korea: what moves the figure

Foreign patients here pay the same surgery fee as Korean patients, and the estimate is given in writing. What changes the number for you is the plan itself:

  • Grafting alone, or with lower lid surgery. Adding fat repositioning or removal makes a longer operation and a different estimate.
  • How many areas. Under the eyes only, or also the cheek groove, temples or forehead.
  • A second session. For a deep hollow, a second, smaller graft may be planned months later, and that means a second trip.
  • Your days in Seoul. Hotel nights and time off work are part of what you spend, even though they aren't on the clinic's estimate.

If you're grafting across more of the face, whether it's planned as one session or two changes the total more than anything else. If your hollow is part of a midface that has dropped, a graft alone may be the wrong purchase.

Ask on WhatsApp what your own plan would cost, and attach the straight-on photo plus one from the side. The deposit and payment method are settled after the consultation, so you can raise those in the same message.

What do Reddit reviews of under eye fat grafting in Korea get right?

Threads you find by searching “under eye fat grafting Korea reddit” tend to repeat three claims.

“Most of the fat melts away.” Partly true. Some of it is reabsorbed in the first months, and the fat that survives behaves like the rest of your body fat. If you gain or lose a lot of weight later, grafted fat can change along with it.

“Lumps are common under the eyes.” Take this one seriously. The lower lid has some of the thinnest skin on the face, so an irregularity that would be invisible on a cheek can show there. It's a known risk, and it's the main reason the amount placed under the eyes is kept small.

“Fat grafting fixes eye bags.” It doesn't. Fat adds volume. A bag is extra volume pushing forward, and it needs repositioning or removal. Threads rarely point out that every review describes someone else's lid, rim and skin. Your own photo tells you more than a thread can.

Results: how much of the graft stays

The swelling right after surgery is not the result. A grafted hollow commonly looks fuller than planned for the first weeks, then settles as the swelling fades and part of the fat is reabsorbed. The contour you see after roughly three months is much closer to what stays.

Fat that survives can last for years, and it ages along with your face. It won't stop the rest of your face from changing, which is why a hollow in your 40s may need a different conversation in your 60s.

What does recovery ask of your trip to Seoul?

In terms of volume, under-eye fat grafting is a small operation. But it's on your face, so the first days are visible. Early on, sunglasses help more than concealer, and many people sleep with their head raised for the first nights.

I won't put a number of days in Korea on this page. The answer changes if lower lid surgery is added, with the anesthesia, and with how quickly your bruising fades. Ask how many days to plan before you book the return flight, and keep a spare day if you can. The clinic is at 635 Nonhyeon-ro, Gangnam-gu, so a hotel nearby keeps the trip to and from the clinic short.

Once you know your travel dates, send them on WhatsApp with a side-on photo of your lower lids. Ask how many days a graft like yours needs before flying. With your dates in the message, you're asking about your trip, not an average one.

Risks and side effects to know

The common reactions are temporary: swelling, bruising, a tight feeling under the eyes and tenderness where the fat was taken. These usually settle over the first weeks. Less common problems include a lump or uneven contour, a result that is too full or not full enough, and fat that doesn't survive in one spot. A 2022 systematic review of facial fat grafting complications in the Journal of Clinical Medicine also describes a rare but serious one: fat entering a blood vessel, which can affect vision or the skin.

This is why small amounts and blunt cannulas are generally used around the eyes. How you heal will differ from someone else, which is one more reason the plan starts from your own face.

In general, you should wait, or talk it through first, if any of these apply:

  • An infection, inflammation or a stye near the eye
  • Filler still present in the area
  • Pregnancy or breastfeeding
  • Blood thinners or a bleeding disorder
  • Very little spare body fat
  • A bulging bag as your main concern

After you fly home, a small firm spot in the first weeks is often swelling and may soften. Take a daylight photo and note when it appeared. Sudden vision change, severe pain, or skin that turns pale or dark over the area is an emergency: go to an emergency department at home first. Before booking, ask what follow-up looks like once you're back, so you have that answer before you need it.

Why the team matters for a graft this close to the eye

The anesthesiologist with you doesn't operate. Their job is to choose the anesthetic approach and watch your blood pressure, pulse and breathing throughout. You can see how photo quotes and bookings work on our photo quote and consultation page.

When you're ready, send one straight-on daylight photo of your under-eye area on WhatsApp, with the year of any filler you've had there. Ask whether that filler needs to be dealt with before a graft. On our site, the surgeon reviews your photos first.

FAQ

Questions patients ask

What is the fat under your eye called in Korean?

The fat pads under the eye are called nunmit jibang, which literally means under-eye fat. Grafting fat into a hollow is usually called nunmit jibang isik. Repositioning the fat pads is nunmit jibang jaebaechi. The three terms can look alike in Korean search results, so check which procedure a page is describing.

Where is the fat taken from for an under-eye graft?

Usually from an area with some spare fat, such as the lower abdomen or inner thigh. Only a small amount is needed for the under-eye area. That donor site is usually bruised and tender for a while, so pack loose clothing.

Will I need a second session?

Not always. Because part of the graft is reabsorbed, a deep hollow is sometimes treated in two smaller sessions rather than one large one. If you're traveling, ask at the consultation whether a second session is likely, because it means a second trip.

Can under-eye fat grafting be combined with lower eyelid surgery on the same trip?

Combining them changes the length of the operation, the estimate and the recovery. Ask about the combined plan, not each part separately.

Is the under-eye area done under general anesthesia?

Not necessarily. The choice is made with the anesthesiologist, who doesn't operate and stays with you through the whole operation.
