You'll know what reviews get right, how recovery runs, which methods exist and what moves the fee, so you can tell what your lower lids and your savings actually call for.
- Moving, removing and adding fat are three separate plans for the lower lid.
- A bag above a groove is the pattern repositioning tends to suit.

Three plastic surgeons share our eye center, and lid thickness, fat and lifting strength get checked before anyone decides whether to cut. One of our three staff anesthesiologists is dedicated to every operation. Foreign patients pay the same surgery fee as Korean patients, with a written quote and an interpreter included.
I wrote this because many people assume a bag has to come out, and on a lid that's already hollow, taking fat out can leave it looking more tired.
Under eye fat repositioning moves bulging lower-lid fat down into the groove beside it. It tends to suit a bag sitting above a hollow, and the fee follows which layers need work, the anesthesia and anything added.
To start, take one straight-on photo in daylight with no makeup, send it on WhatsApp, and ask: is this fat, a groove or skin? Then you're asking about your own lids, and you can compare any advice against that answer.

Moving, removing or adding fat: three different operations
In short, these three get lumped together as eye bag surgery, and they solve different problems.

| Plan | What happens | Tends to suit |
|---|---|---|
| Repositioning | Bulging fat is released and draped into the groove | A bag with a hollow right below it |
| Removal | Some of the bulging fat is taken out | A full bag with little hollow beneath |
| Fat grafting | Your own fat is injected into hollow areas | Hollowness with little bulge to move |
Each can be done through the inside of the lid or through a cut just below the lashes. The inner route leaves no outside scar and doesn't address extra skin. The outer route allows skin to be trimmed and lid support to be tightened, and it's linked to more risk of the lid pulling down. The American Society of Plastic Surgeons describes both routes on its eyelid surgery page. For stay length and the anatomy in more depth, I've written separately about bags, hollows and planning your stay.
Does a bag always have to come out?
No. What looks like too much fat is often the same fat, sitting forward and made more obvious by the groove below it. As the tissues holding it loosen with age, it pushes out, and the light catches the step between bag and hollow. Remove that fat and the step can shrink, but the hollow stays, and may deepen.
That's the reasoning behind moving the fat, since it fills the groove with tissue you already have. It isn't right for everyone, though. Say you're 28, with a family tendency to full lower lids, firm skin and almost no groove. There's little hollow to fill, so moving fat into it wouldn't do much, and a small removal through the inner lid may be the plan I'd lean toward. Now say you're 45, with a deep groove and some loose skin. I'd want the fat moved, and I'd talk about the skin too.
Also, a dark circle and a bag are different things. If the darkness comes from pigment or from vessels showing through thin skin, moving fat usually changes it only partly. I'd rather you hear that before you've spent anything.
What do reviews get right, and what can't a balm do?
Reviews of under eye treatments mix products, injections and surgery, so here's how the usual claims hold up.
"Filler gives the same result without surgery." Partly. Filler can soften a groove, but it adds volume without moving the bulge above it. It can also drift or sit visibly in thin lid skin, and under eye filler migration is a common reason people start looking at surgery. Tell your surgeon if you've had any, including when and which type.
"An under eye balm got rid of my bags." A balm, concealer or cream can change how skin looks that day. None of them can move fat that sits behind a membrane, which is why glowing balm reviews and surgical advice so rarely describe the same problem.
"Once it's done, it's done for life." Too strong. Repositioned fat generally stays, but skin and tissue go on aging, so the area will keep changing over the years.
When does fat grafting under the eyes belong in the plan?
When hollowness reaches beyond what your own lid fat can fill, such as along the cheek or the outer lid. Fat is usually taken from the abdomen or thigh, purified, then injected in thin layers. Here that sequence is harvest, purify and layered injection, and what counts is how much of the fat stays, more than how much went in.

Two cautions apply. Lower lid skin is thin, so small lumps can show. And some injected fat doesn't survive, so a touch-up is sometimes discussed. If you've read about how fat transfer behaves in the breast, the survival question works the same way, on a far smaller scale. The harvest site is a minor part of this operation, and a different job from liposuction for body shape, so don't plan on seeing a change in the donor area.
How long is recovery, and can it fit your leave?
For many people, the obvious bruising and swelling settle over one to two weeks, with subtler swelling lasting longer. Added fat grafting usually means swelling at the lids and some soreness at the donor site as well. Your eyes may feel dry or gritty for a while.
Say you have a two-week break and a tight budget. Book the surgery near the start, plan your sightseeing for sunglasses and short walks, and ask your surgeon how many days to keep before your flight. The US Centers for Disease Control and Prevention warns that flying soon after surgery raises the risk of blood clots. Leave a spare day if you can, because a rebooked flight is its own expense.
Under eye fat repositioning Korea cost: what moves your figure
The under eye fat repositioning Korea cost for your lids comes from five things:
- Which layers need work. Fat alone, or fat plus skin and lid support.
- The approach. The inner route or a cut below the lashes.
- The anesthesia. Local with sedation or general, depending on the scope.
- Added fat grafting. A second site and more surgical time.
- Your days in Seoul. Nights in a hotel and follow-up visits belong in your budget.
If money is the limit, ask which items your lids actually need, since I recommend only the operations that are necessary. To get a figure for your case, send your photo on WhatsApp and ask what it would cost. You can see our doctors and procedure categories on VG Plastic Surgery's English home page before you write.
What can go wrong once you're home?
Most concerns surface in the first weeks, often after your flight, so it helps to know the warning signs and who you'd call.
Risks and side effects to know
Common and usually temporary: bruising, swelling, dry or irritated eyes, tearing, blurry vision for a short time, and small firm areas where fat was placed. Healing differs from person to person.
Less common: uneven fat, a bag that remains or returns, visible lumps, infection, and the lower lid pulling down or the eye not closing fully. The American Society of Plastic Surgeons lists dry eyes, difficulty closing the eyes and lid changes among the risks of eyelid surgery. Bleeding behind the eye and vision loss are rare but serious.
These are generally reasons to wait or skip surgery: untreated dry eye, a lax lower lid that hasn't been assessed, an active eye infection, blood thinners that can't be paused safely, uncontrolled thyroid eye problems, pregnancy or breastfeeding, and filler still sitting in the area.
Sudden eye pain, a tense swollen eye, or any drop in vision needs an emergency room the same day, wherever you are. For a slower problem such as dryness or a lid that won't close, see a local eye doctor. The CDC advises keeping copies of your medical records and arranging follow-up at home before you travel, so have your operative note on your phone.
If you've had filler or earlier eyelid surgery, note the date and type in a WhatsApp message, and ask how that changes the plan. Those two details are easy to forget and hard to work out from a photo.

One photo is enough to begin
You can ask before booking anything. Send one daylight photo on WhatsApp with your travel month, and ask: what would moving this fat cost in my case, and do I need anything else? When you come to Seoul, the doctor examines your lids and recommends the treatment that fits, which may be less than you came prepared to pay for.

FAQ
Questions patients ask
How is repositioning different from eye bag removal?
Removal takes fat out, while repositioning keeps it and moves it into the groove below. A bag with a hollow beneath often suits moving, and a full lid with little hollow may suit a small removal.
Will I have a visible scar?
With the inner-lid route, the cut is on the inside. A cut below the lashes usually fades into a fine line over months. Scarring varies from person to person, so ask which route your lids need.
Can I have this after under eye filler?
Often, but tell your surgeon the type and date first. Leftover filler can change how the tissue looks and behaves in surgery, and it may need dealing with before fat is moved.
How do I work out the under eye fat repositioning cost in Korea for my budget?
List your hotel nights, flights and leave next to the surgical quote, which should name the approach, anesthesia and any fat grafting. Foreign patients here pay the same surgery fee as Korean patients, and the quote is written.
Does PRF gel or a Korean under eye treatment replace surgery?
For a true fat bulge, no injection or topical product moves the fat. They may change skin or fill a mild hollow. We're a surgical clinic, so ask a dermatologist about non-surgical options.
